Payment for services are due at the time of service. If you have insurance coverage, we will provide you with a Receipt or Superbill that you can submit to your insurance company. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
What is my health insurance coverage and benefits?
What is my deductible and has it been met?
Do I have a co-pay? If so, how much is it?
What is my wellness visit coverage?
Cash, debit, Visa and MasterCard credit cards are accepted for payment.
In an effort to better serve our patients appointments are not necessary and we will gladly provide consults on a Walk-In basis.
